Project Duration : 13 Weeks/Contract
Shift Timing : Days
Job Summary
We are seeking a compassionate and dedicated Social Worker to support individuals and families in navigating challenging life situations. The Social Worker will assess client needs, develop care plans, and coordinate with community resources to improve overall well-being. The ideal candidate will have strong interpersonal skills, a deep understanding of social systems, and a commitment to advocating for vulnerable populations.
Key Responsibilities:
- Conduct psychosocial assessments and develop individualized service plans
- Provide counseling and crisis intervention to individuals and families
- Connect clients with community resources such as housing, healthcare, and financial assistance
- Collaborate with healthcare professionals, schools, and legal systems to support clients
- Maintain accurate and confidential case records and documentation
- Advocate for client rights and access to services
- Monitor and evaluate client progress, adjusting services as needed.
Qualifications:
- MA license as Licensed Clinical Social Worker (LCSW)
- Or Licensed Independent Clinical Social Worker (LICSW)
- Previous Two years’ experience
- Preferred: Acute care setting experience.
