Job description:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our long-term care team. The ideal candidate will have recent experience in a long-term care (LTC) setting and be committed to providing high-quality patient care.

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our long-term care team. The ideal candidate will have recent experience in a long-term care (LTC) setting and be committed to providing high-quality patient care.
Key Responsibilities:
- Provide direct nursing care to residents in accordance with established care plans
- Administer medications and treatments as prescribed by physicians
- Monitor patient conditions and report any changes to the healthcare team
- Maintain accurate and timely documentation in the electronic medical record (EMR) system
- Collaborate with interdisciplinary team members to ensure comprehensive resident care
- Assist with admissions, discharges, and transfers as needed
- Adhere to all facility policies, procedures, and regulatory requirements
- May be required to float between units based on staffing needs
- Assume charge nurse responsibilities for assigned unit as required
Qualifications:
- Active LPN or RN license in good standing
- Minimum of 2 years of recent experience in a long-term care (LTC) setting
- Current Basic Life Support (BLS) certification
- Experience with electronic medical records (EMR) systems preferred
- Strong clinical, organizational, and communication skills
- Ability to work effectively in a team-oriented environment
Work Environment:
- Long-term care facility setting
- May require flexibility to work across multiple units/floors
