Position Details:
RN Hospice Case Manager­ Barre, VT
Location                            : Barre, VT 05641
Project Duration           : 13 weeks
Shift                                    : Sunday-Thursday (M-F first week to facilitate orientation)
 
Job Description:
Experience/Education/Qualifications

• Experience with hospice case management required
• Experience providing hospice care in a home setting required.
• Prior experience with rural travel preferred.
• Experience with McKesson/Netsmart EMR preferred
• Work schedule Sunday-Thursday
