Responsibilities:
- Conduct comprehensive assessments and develop individualized care plans for high-risk patients
- Coordinate services and referrals with healthcare providers, behavioral health, substance use, and community-based resources
- Monitor patient progress, modify care plans as needed, and respond to emerging needs
- Perform utilization review for inpatient and outpatient services
- Evaluate medical necessity for referrals to specialty services not offered in-house
- Collaborate with physicians, nurses, social workers, and care coordinators to support continuity of care
- Promote efficient use of healthcare resources and patient-centered care practices
- Document all care coordination activities accurately in the electronic health record
Core Competencies:
- Associate Degree in Nursing required
- Bachelor of Science in Nursing (BSN) must be obtained within 2 years of hire
- Unrestricted RN license valid in any U.S. state or territory
- Current Basic Life Support (BLS) certification by the American Heart Association
- Minimum 3 years of clinical care experience
- At least 1 year of travel nursing experience
- Prior experience in case management, discharge planning, or utilization review
- Proficiency in using electronic health records (EHR systems)
