We have Contract role Hospice/Medical Social Worker for our client at Albany, OR. Please let me know if you or any of your friends would be interested in this position.
 
Position Details:
Hospice/Medical Social Worker- Albany, OR
Location                            :  Albany, OR 97321
Project Duration           : 13 weeks
Shift                                    : Monday to Friday (8am – 4:30pm)

Job Description:

• The Social Worker will collaborate with an exemplary interdisciplinary team dedicated to providing compassionate, high-quality care to patients in the community.
• This role focuses on supporting patients through education, resources, and care that respect their individual goals and wishes.

 
Key Responsibilities:

• Provide psychosocial assessments, counseling, and support to patients and families.
• Participate in interdisciplinary care planning and deliver hospice-focused social work services.
• Assist with coping strategies, resource coordination, and end-of-life planning.
• Maintain accurate documentation and adhere to regulatory and organizational standards.
• Travel to patient homes and care facilities across assigned counties.

 
Required:

• Oregon LMSW (Master’s-Level Social Worker) or CSWA (Clinical Social Work Associate) licensure.
• Healthcare Provider Level BLS certification.
• Valid driver’s license and personal vehicle with required auto liability/property damage insurance; vehicle must be maintained in good working condition.

 
Preferred:

• Oregon LCSW (Licensed Clinical Social Worker).
• Previous experience as a social worker in a hospital, hospice, or medical setting.
