Location : Fort Defiance AZ, 86504
Project Duration : 13 weeks
Pay range : $53-$57 an hour
Shift : Day
Job description:
The Clinic Care Coordinator (Case Management) supports patient care delivery within the Surgery Outpatient Clinic by working collaboratively with nursing staff, physicians, and interdisciplinary teams. This role is responsible for coordinating patient care plans, ensuring continuity of care, and promoting efficient, high-quality clinical outcomes.
Key Responsibilities:
• Coordinate patient care activities in collaboration with nurses, surgeons, and healthcare providers.
• Develop, implement, and monitor individualized care plans for outpatient surgical patients.
• Facilitate communication between patients, families, and the care team.
• Ensure timely scheduling, follow-ups, and referrals as needed.
• Monitor patient progress and address barriers to care.
• Support discharge planning and continuity of care post-procedure.
• Maintain accurate documentation in accordance with clinical and regulatory standards.
• Promote patient education and engagement throughout the care process.
Qualifications:
• Experience in case management, care coordination, or a related healthcare role preferred.
• Strong communication, organizational, and problem-solving skills.
• Ability to work effectively in a collaborative, fast-paced outpatient setting.
